①Agreement Form
②Registration form (can be downloaded on this site)
③Size table (can be downloaded on this site)
④Passport
⑤Residence card (Front & back)
⑥Driver’s License ( Only If you have )
⑦Profile photo (Picture that shows your face・whole body )7~10 pictures are required .
Pictures such as suits, casual, and sporty etc…
There are three different papers
1. Registration form
2. Size table
3. Agreement form. (model registration advance confirmation)
Agreement form will be signed in the office when coming for the
explanation. Please come with your my number card and Bank Book or
Bank Card.
